Ahca User Registration Agreement

The first registration for LTC Trend Tracker℠ can be completed in a few steps. (Please note that the registration of multicentre/community organisations must be completed by the company`s headquarters). AHCA/NCAL must verify the membership and ownership status of the center/community before the organization has access to LTC Trend Tracker℠. We may request your CMS 855A form if we cannot verify the property. Please provide the contact information of an authorized representative of the organization authorized to verify and accept the LTC Trend Tracker Acceptance Agreement (see below). 3. We control and monitor access to the LTCTT using authorized user accounts and passwords. You represent and warrant that you have the authority to enter into this agreement on behalf of your member organization/multi-facility member organization. 9. You are prohibited from transmitting membership data in the form of a message or otherwise to any part of the LTCTT that contains or discloses personal data, including patient-specific information. However, we may use and provide member data from other users and member organizations for research and general information purposes, as described above.

AHCA reserves the right to require you and each of your other member organizations, at their discretion, to execute counterparty agreements where such instruments are necessary for AHCA`s compliance with applicable law. Your failure to enter into such an agreement may result in AHCA suspending and/or term ending your access to the LTCTT.
